Suburban Metro Area Continuum of Care
The Suburban Metro Area Continuum of Care program provides permanent supportive housing services for single adults that pay rent only throughout the program.
The SMACC program aims to create a more effective and efficient system for preventing and addressing homelessness.
Who Can Get Services
To be eligible, you must meet the following criteria:

Have no income

Be a Minnesota resident or household
-
Be a resident in one of the following counties:

Anoka
-
Carver
-
Dakota
-
Ramsey
-
Scott
-
Washington
-
-
Experiencing a housing crisis, chronic homelessness or disability

Single individual
How To Get Services
STEP 1: Get an Assessment from a...

Coordinated entry assessor in your area and they will conduct a screening and assessment with you to determine if you are eligible for supportive housing.
STEP 2: If you qualify...

Your name and housing assessment information will be added to your county's Housing Priority List.
STEP 3: Stay in touch...

Keep your assessor updated regularly while you are waiting to let them know if your contact information or living situation changes.

Continue to look for housing on your own, as program placement through Coordinated Entry is not guaranteed.
STEP 4: Notifications...

When you are notified by a housing program that they have an opening for you, you will meet with them to be sure that is'a good match for you!
