Suburban Metro Area Continuum of Care

The Suburban Metro Area Continuum of Care program provides permanent supportive housing services for single adults that pay rent only throughout the program. 

​

The SMACC program aims to create a more effective and efficient system for preventing and addressing homelessness.

Who Can Get Services

To be eligible, you must meet the following criteria:​

  • Be a Minnesota resident or household

  • Be a resident in one of the following counties:

    • Anoka​

    • Carver

    • Dakota

    • Ramsey

    • Scott

    • Washington

  • Experiencing a housing crisis, chronic homelessness or disability​

  • Single individual

How To Get Services

STEP 1: Get an Assessment from a...

  • Coordinated entry assessor in your area and they will conduct a screening and assessment with you to determine if you are eligible for supportive housing.

​

STEP 2: If you qualify...

  • Your name and housing assessment information will be added to your county's Housing Priority List.

​

STEP 3: Stay in touch...

  • Keep your assessor updated regularly while you are waiting to let them know if your contact information or living situation changes. 

  • Continue to look for housing on your own, as program placement through Coordinated Entry is not guaranteed.

STEP 4: Notifications...

  • When you are notified by a housing program that they have an opening for you, you will meet with them to be sure that is'a good match for you!
